HOW IT WORKS?
BY APPOINTMENT ONLY
1.Call the SHS for an appointment at (562) 938-4210 or
for any questions.
2. Park In a designated space at the Pacific Coast Campus Parking
Lot 1
(Enter from Orange Street).
3. Call the number listed on the sign and let us know you are
here. Students must stay In cars-Please do not come to the
clinic.
4. Face Covering Required
*There will be a walk-up table for students who do not
arrive In cars.

2. Click on point and click
3. Confirm birthday
4. Click forms tab
