Measles (Rubeola), Mumps & Rubella – One of the following is required: 2 vaccinations OR Positive antibody titer for all 3 components (lab report required)

Professional Liability Insurance – Provide documentation of your current Professional Liability Insurance. Coverage limits must be a minimum of $1,000,000 per incident and $3,000,000 aggregate. The renewal date will be based on the expiration of your policy.

Varicella (Chicken Pox) – One of the following is required: · 2 vaccinations OR · Positive antibody titer (lab report required

Hepatitis B – One of the following is required: · 3 vaccinations OR · Positive antibody titer (lab report required

Tuberculosis (TB) – One of the following completed within the past 12 months is required: · 2 step TB skin test (administered 1-3 weeks apart) OR · QuantiFERON Gold blood test (lab report required) OR · If positive results, upload a clear chest x-ray. The renewal date will be set for 1 year. Upon renewal, one of the following is required: · 1 step TB skin test OR · QuantiFERON Gold blood test (lab report required) OR · If previous positive results, a the school form TB questionnaire (available for download) will be required

Influenza (Flu) – Submit documentation of a flu vaccine administered during the current flu season

CPR Certification – American Heart Association BLS Provider CPR certification

COVID-19 Vaccination – Original vaccine + 1 booster OR One of the following 2023-2024 COVID-19 vaccinations is required: · ONE dose of Moderna OR · ONE dose of Pfizer- BioNTech OR · AT LEAST ONE dose of Novavax
